It’s a deeply human endeavor that requires judgment, communication, ethical reasoning, and the ability to navigate profound uncertainty while holding someone’s hand through the scariest moments of their life.
Consider what actually happens in a doctor’s office. A patient comes in with vague symptoms that could indicate anything from anxiety to cancer. The doctor doesn’t just run through a diagnostic checklist. They observe body language, pick up on what isn’t being said, factor in the patient’s unique circumstances and values, and make decisions with incomplete information while managing risk, cost, and quality of life considerations. They explain complex medical concepts in ways that resonate with each individual patient. They deliver devastating news with compassion. They convince skeptical patients to follow treatment plans. They coordinate care across multiple specialists. They adapt on the fly when the textbook answer doesn’t fit the actual human in front of them.
AI can’t do most of that. Not because the technology is a few years away from being good enough, but because these tasks require the kind of flexible, contextual intelligence that we’re nowhere close to replicating. The models we call “artificial intelligence” are sophisticated statistical engines that excel at specific, well-defined tasks. Medicine is the opposite: a sprawling domain where almost nothing is well-defined and context is everything.
Even in the narrow areas where AI performs impressively, like radiology, we’re not seeing replacement but rather integration. Radiologists are using AI as a second pair of eyes, a tool that flags areas of concern for human review. This is how most medical AI will work for the foreseeable future. The more sophisticated our medical technology becomes, the more education is required to use it properly. An AI diagnostic system powerful enough to rival human doctors would be complex enough that operating it effectively would require deep medical knowledge. You’d essentially need a doctor to properly use the doctor-replacing AI.
Think about aviation. We have autopilot systems that can handle most of the routine flying. But we still require pilots to have extensive training and education, precisely because the automated systems are so complex and because human judgment is essential when things don’t go according to plan. The same principle applies to medicine, only more so, because human bodies are infinitely more variable than flight paths.
The bachelor’s degree requirement isn’t just bureaucratic gatekeeping. It represents the foundation of critical thinking, scientific literacy, and general knowledge that you need to make sense of complex systems and handle novel situations. Medical school builds on that foundation. Even if AI handles routine diagnoses, someone needs four years of undergraduate education plus medical training to understand when the routine diagnosis doesn’t fit, what the differential diagnoses might be, what the trade-offs are between treatment options, and how to communicate all of this to a frightened patient.
The real future of AI in medicine isn’t replacement but transformation. Doctors will spend less time on tasks that AI can handle and more time on the irreducibly human aspects of care. They’ll use AI tools to be more efficient and accurate, much like they currently use X-rays, blood tests, and electronic health records. The job will evolve, as all jobs do with technology, but the core requirement for highly educated, compassionate professionals who can think critically and connect with patients will remain.

The skills that make a good doctor, judgment, empathy, communication, ethical reasoning, adaptability, are exactly the skills that AI struggles with most.
